In order to advance a well-rounded population health strategy and achieve health equity, public health agencies must develop a balanced set of strategies and programs that go beyond the control of disease and prevention of physical health conditions. The time is now to build a more prepared, resilient, and proactive public health system at every level to achieve this vision of health equity.
The Pathways to Population Health Equity Framework and a set of accompanying implementation tools, released in April 2022, help public health practitioners move upstream to address the root causes and social determinants of health in alignment with their community. This webinar will equip participants with a tactical set of tools to:
• Establish a common language, purpose, and political will to address population health.
• Work effectively with partners to build community-centered interventions.
